Die Wahrnehmung von Menschen mit Behinderungen unterlag während des letzten Jahrhunderts bis heute einem stetigen Wandel hin zu einem nun rights based geprägten Zugang. Das soziale Modell, welches die Behinderung nicht mehr als „Fehler“ des Individuums identifiziert, sondern als soziale Interaktionskonsequenz von physischen Einschränkungen, Aktivi-tätseinschränkungen und sozialer Exklusion, erlebte in der Convention on the Rights of Persons with Disabilities der Vereinten Nationen eine noch nie dagewesene Aufwertung. Im Zu-ge der veränderten Wahrnehmung von Menschen mit Behinderung erlebten auch die medizinischen Professionen der Rehabilitation einen Bedeutungsaufschwung. In dieser Arbeit werden die Konsequenzen und das Wirken der neuen Aufwertung anhand der Interventionen vor allem zweier internationaler NGOs (Christoffel Blinden Mission, Handicap International) und einer lokalen haitianischen NGO (Healing Hands for Haiti International Foundation) als Antwort auf das Erdbeben in Haiti 2010 diskutiert. Um die Darstellung des Interventionskom-plexes zumindest exemplarisch zu ermöglichen, liegt der Fokus dieser Arbeit auf der Implementierung von Physiotherapie in die Gesundheitsversorgung Haitis. Der dabei verwendete systemtheoretische Zugang soll helfen, die Interaktionen der verschiedenen Akteur_innen sowie den Einfluss des globalen Diskurses und der lokalen Gegebenheiten zu verstehen. Abschließend wird aufgrund dieser Informationen versucht, den Nachhaltigkeitswert der Interventionen zu beurteilen. Das ungleich größere Ziel der Arbeit liegt aber im Sichtbarmachen der allgemeinen Mechanismen, die hinter verschiedenen immer noch aktuellen Problematiken der Entwicklungszusammenarbeit stehen, um sie (be)greifbar und in weiterer Folge veränderbar zu machen.

The aim of this work was to reveal the interactions of international discourse, local circumstances, and the specific interventions of various actors in an effort to create a sustainable change in the Haitian health system in favour of people with Disabilities. This author chose to use a systems theory approach to try to uncover the interaction mechanism and answer questions of sustainability. The first part of this work illuminates the international discourse on the Rights of People with Disabilities. The discourse has undergone constant change before culminating in the broad acceptance of the social model and its anticipation by the Convention on the Rights of Persons with Disabilities. The perception was explained due to the promotion of the rights based capability approach and its reflection by various policy papers of the international community.
In order to evaluate the sustainability of taken action it is vital to examine the local context, including the Haitian perception of people with disabilities, locally existing concepts of disease and the general Haitian health-system as well as its actors.
Furthermore the massive earthquake rocking Haiti in January 2010 is perceived as an extrinsic factor, putting Haiti and its on-going crisis back into the international spotlight. Moreover the massive destruction attracted uncountable international actors who flooded the country with the best intentions and goodwill. Linked to the international discourse on people with disabilities it was especially people with disabilities in general, and even more specificly, people with amputations and spinal cord injuries upon who´s specific needs international actors attention was centred.
In the case of Haiti though, it was also not only classic non-profit actors who were entering the stage of the rehabilitation sector but also big and financially well equipped companies, e.g. Hanger Inc., a North-American orthopaedic devices producing enterprise which opened its own clinic in Deschappelles after the earthquake together with other NGOs.
Through study and reflection on the international discourse, as well as the local circumstances, and the destruction resulting from the massive earthquake; this work aimed to create a working framework that can be used to evaluate the sustainability of the taken actions by two international NGOs namely Christoffel Blinden Mission and Handicap International and one local Haitian NGO called Healing Hands for Haiti International Foundation.
The use of an adapted sustainability evaluation framework showed that the interventions of the international NGOs were only partly sustainable, though with big potential. Important points of criticism were the missing plan on sustaining provision of physiotherapy when and if other crises were to hit the country, and the training programmes which were provided without national accepted degrees and were lacking of at least national regulations and the disregard of rural areas by the interventions.
The broader goal of this work was to reveal problem generating mechanisms of international co-operations and their consequences in order to illuminate and criticise them. These general mechanisms result in the proliferation of NGOs and are linked to the lack of government accountancy. Another identified side-effect was the missing co-ordination of actors, subtle economic interests and the problem of flag-waving international NGOs longing for attention. Conversely some trends confirm the positively changed perception of people with disabilities within the society, which may lead to future full “inclusion” of people with disabilities as equal participants of society in Haiti as well as globally.
