Transportproteine aus der Familie der Organischen Anion Transportierenden Polypeptide (OATP Familie, SLC21) regulieren den Transport und die Aufnahme unter anderem von Hormonen, Prostaglandinen, cyclischen Nukleotiden und verschiedenen Chemotherapeutika. Daher könnte OATP4A1 als Angriffspunkt für neue therapeutische Ansätze zur Therapie von Tumoren wichtig sein.
Die Expression von OATP4A1 wurde mittels quantitativer mikroskopischer Bildverarbeitungstechnik an Formaldehyd- fixierten und Paraffin-eingebetteten Gewebeproben erforscht. Dafür wurden Gewebeschnitte von 185 Patienten mit kolorektalem Karzinom (UICC Stadium I/II) untersucht und die Daten mit 18 Divertikulitispatienten verglichen.
Die Schnitte wurden mittels Immunhistochemie gefärbt und die Färbeintensität, wie auch der Prozentsatz an gefärbten Zellen mit dem Programm HistoQuest® ausgewertet, wobei zwischen Tumor-, Immun- und Stromazellen unterschieden wurde. OATP4A1 Membran- und Cytosolfärbung wurde sowohl in Tumorzellen, als auch in Immunzellen gefunden. Bei manchen Tumorpatienten war vereinzelt auch eine Kernfärbung zu sehen.
Ein höherer immunreaktiver Wert (immunoreactive score IRS) wurde bei Kolonkarzinompatienten, welche kein Rezidiv innerhalb von fünf Jahren erlitten, festgestellt, im Vergleich zu Patienten mit Rezidiv und normalen Epithelzellen. Die höchste Expression von OATP4A1 wurde in Immunzellen von Patienten mit einer besseren Prognose gefunden (kein Rezidiv innerhalb von 5 Jahren). Ähnliche Ergebnisse wurden auch für Tumor-, und Stromazellen beobachtet. Im Vergleich zu Epithelzellen in Divertikulitisproben zeigte sich nur eine sehr schwache Expression von OATP4A1.
Zur Identifizierung von Immunzellen, welche OATP4A1 exprimieren, wurden Doppelimmunfluoreszenzfärbungen mit geeigneten Markern durchgeführt. OATP4A1 positive Immunzellen wurden in Subpopulationen von CD45+ Leukozyten, CD3+ T- und CD20+ B-Lymphozyten, Monozyten, Makrophagen, dendritischen Zellen und CD34+ Vorläuferstammzellen gefunden.
Die erhöhte Expression von OATP4A1 sowohl in Tumor-, als auch in Immunzellen in Kolonkarzinompatienten weist darauf hin, dass die Aufnahme von endogenen Substraten wie Prostaglandinen, cyclischem AMP, Peptiden oder Arzneistoffen, möglicherweise für Zellproliferation und das Tumorwachstum wichtig sein könnte.

The organic anion transporting polypeptide transporter OATP4A1, a member from the SLC 21-membrane transporter family may be important for tumor progression and the therapeutic response to chemotherapeutic drugs, due to its role in the transport of endogenous metabolites, xenobiotics and drugs. By transporting hormones, prostaglandins, cyclic nucleotides and chemotherapeutic agents into the cells, OATP4A1 influences the cellular levels of these compounds and their therapeutic efficacy.
Therefore, we investigated and defined the expression and potential prognostic value of OATP4A1 in 185 specimens from early colorectal cancer (UICC Stage I/II) and did comparative studies in 18 non-malignant diverticulitis samples.
OATP4A1 levels were evaluated by quantitative microscopic image analysis on paraffin-embedded tissue sections stained by immunohistochemistry (IHC). Quantification of the immunoreactive score (IRS) of OATP4A1 was done separately, on tumor cells, stroma and immune cells, using the HistoQuest® software. Cell identification was done automatically based on algorithms for cell identification from nuclei stained with hematoxylin.
OATP4A1 was detected in the membrane and cytosol of colon cancer and immune cells. A higher immunoreactive score (IRS calculated from the percentage of stained cells x staining intensity) was found in the cancer areas than in the normal mucosa in the adjacent non-cancerous tissue. However, highest OATP4A1 levels were observed in immune cells in the tumors from patients without an early relapse compared to patients with a relapse within five years. Similar values for OATP4A1 were observed in tumor and stroma cells, present at a higher number in tumors of patients with a better prognosis. In normal mucosa and in the mucosa of diverticulitis patients, OATP4A1 was largely absent.
To identify OATP4A1-positive cell types, double-immunofluorescence staining (IF) with appropriate cellular markers was done. OATP4A1 positive immune cells were identified in subpopulations of CD45+ leukocytes, CD3+ T- and CD20+ B-lymphocytes, monocytes, macrophages, dendritic cells and CD34+ precursor cells.
Abundant expression of OATP4A1 in cancer and immune cells in colonic disease led to the conclusion that by providing endogenous substrates (prostaglandins, cyclic nucleotides or peptides) to cells, the transporter may contribute to cell proliferation and differentiation.
