Als ein Hauptproduktionsort für Calcitriol bzw. als ein Zielorgan für Parathormon spielt die Niere eine essentielle Rolle in der Kalziumhomöostase. Dazu führt eine typisch verstärkte Phosphatretention bei der Niereninsuffizienz zu einem Parathormonanstieg, welcher bis zu einem sekundären Hyperparathyreoidismus progradieren kann, mit katabolischen Auswirkungen auf das Skelett und einer konsequent gesteigerten Frakturanfälligkeit. Eine Niereninsuffizienz dürfte somit mit der Pathophysiologie der Osteoporose assoziiert sein.
Schleifendiuretika werden mit einer verstärkten Kalziumausscheidung assoziiert, wodurch – in Abhängigkeit von der Kalziumzufuhr und dem Vitamin-D-Status – eine negative Kalzium-Balance ausgelöst werden kann. Somit stehen auch Schleifendiuretika in Zusammenhang mit einem Parathormon-vermittelt, verstärkten Knochenabbau und einem potentiell erhöhten Frakturrisiko. Diese Diplomarbeit versucht die Frage zu beantworten: Gibt es Hinweise auf eine mögliche Potenzierung der Skelettauswirkungen von den Schleifendiuretika bei einer chronischen Niereninsuffizienz und/oder Vitamin-D-Insuffizienz? Dazu wurde ein geriatrisches Patientenkollektiv auf die Prävalenz der Risikofaktoren geprüft, mit einer anschließen Untersuchung ihrer Auswirkungen auf das Parathormon sowie die Knochenparameter. Da eine Koexistenz mehrerer Risikofaktoren bei den Patienten im Kollektiv auch keine Seltenheit war, könnte ein Vergleich der auf dieser Basis geformten Patientengruppen durchgeführt werden, wodurch eindeutige Hinweise einer potenzierten Wirkung – sowohl auf den Parathormon-Spiegel, als auch auf die Knochenparameter – bei jedem zusätzlichen Risikofaktor gezeigt werden konnten, mit einem besonders ausgeprägten kumulativen Effekt in der Gruppe mit allen drei untersuchten Risikofaktoren. Zusammenfassend wurde auf die Notwendigkeit einer weitergreifenden Optimierung des Vitamin-D-Status eingegangen, mit besonderem Augenmerk auf den postmenopausalen Frauen sowie älteren Männern mit eingeschränkter Nierenfunktion und/oder einer prolongierten Schleifendiuretikatherapie.

Kidney is not only a target organ of parathormon action but the main site of the vitamin D activation as well. This explains the kidney’s critical role in the maintaining of normal calcium metabolism and a possible association of chronic kidney disease with the pathogenesis of osteoporosis. The main mechanism in CKD leading to a bone loss and a potential increase of bone fragility is the typically elevated retention of phosphate, with a subsequent stimulation of parathormon secretion that progresses to secondary hyperparathyroidism and a high bone turnover. Loop diuretics are one of the most often prescribed drug groups that are related with the increased calcium excretion. Depending on individual calcium ingestion rate and the status of vitamin D, the prolonged administration of loop diuretics can lead to a negative calcium balance and a subsequent stimulation of parathormon secretion, which rebalances the level of ionic calcium in blood by dissolving it from the bone matrix. Thus, loop diuretics are associated with bone loss and a potentially elevated risk of fragility fractures by a somewhat similar mechanism as in the CKD, with elevated levels of parathormon causing an increased bone turnover. The goal of this work is answering the question: is there a sign of potentiated effects of loop diuretics on bone metabolism is association with the CKD and/or the vitamin D insufficiency? This included a study of the prevalence and the effects of these risk factors on parathormon and bone parameters in a population of hospitalized elderly people. Further, the varying number of coexisting risk factors was used for the classifying of patients in groups, which were than compared to each other in order to verify the differences regarding parathormon level and bone parameters. Indeed, the shown differences between these groups were substantial, with the noticeable effect with every one additional risk factor and the most drastic cumulative effect shown in the group with all three coexisting risk factors. In conclusion: There might be a different strategy needed to assure wide range optimizing of the vitamin D status. This could be of major importance for postmenopausal women and elderly men on prolonged therapy with loop diuretics – especially in case they have a developed CKD.
