Hintergrund: Die Anzahl und Verweildauer geriatrischer Patient:innen im Krankenhaus nimmt aufgrund der Alterung der Gesellschaft und der damit einhergehenden Multimorbidität sowie dem Risiko einer Pflegebedürftigkeit zu. Um geriatrischen Patient:innen eine adäquate Behandlung zu ermöglichen, bedarf es eines ganzheitlichen Ansatzes und interprofessioneller Versorgung. Die Integration einer solchen Arbeitsmethodik ist in der klinischen Praxis mit Hürden und Hindernissen verbunden. Ziel: Ziel dieses Forschungsvorhabens ist es aufzuzeigen, wie interprofessionelle Zusammenarbeit im Rahmen der Versorgung geriatrischer Patient:innen im Akutsetting aus Sicht der involvierten Professionen funktionieren kann. Methode: Die Forschungsfrage wird unter Anwendung eines qualitativen Forschungsansatzes beantwortet. Dazu wurden eine systematische Literaturrecherche, Einzelinterviews und Fokusgruppeninterviews mit Vertreter:innen der Professionen Medizin, Pflege und Physiotherapie, sowie mit weiteren Gesundheitsberufen im Kontext von „Good-Practice“-Fällen durchgeführt. Ergebnisse: Spezifische Aspekte interprofessioneller Zusammenarbeit im Rahmen der geriatrischen Patient:innenversorgung begründen sich durch das Vorliegen von Multimorbidität, kognitiven Einschränkungen sowie einer verminderten Selbstständigkeit seitens der Patient:innen. In einem beständigen multiprofessionellen Betreuungsteam gilt es, eine gemeinsame Zielsetzung zu vereinbaren und durch strukturierte Tagespläne umzusetzen. Der Faktor Kommunikation nimmt aufgrund der Charakteristika geriatrischer Patient:innen eine besondere Rolle ein. Es bedarf einer Teamdynamik, welche jedes Mitglied ernst nimmt und unterstützt um adäquat auf herausfordernde Situation in der Patient:innenversorgung reagieren zu können. Schlussfolgerung: Trotz literaturbasiertem Wissen bezüglich der Einflussfaktoren interprofessioneller Zusammenarbeit im klinischen Setting bedarf es in der Praxis der kontinuierlichen Reflexion patient:innenspezifischer Merkmale und Bedürfnisse, um die Zusammenarbeit im interprofessionellen Team effektiv und effizient gestalten zu können.

Background: The number and length of stay of geriatric patients in hospitals is increasing due to the ageing of society and the associated multimorbidity and risk of care dependency. In order to provide geriatric patients with adequate treatment, a holistic approach and interprofessional care is required. The integration of such a work methodology is associated with hurdles and obstacles in clinical practice. Aim: The aim of this research project is to show how interprofessional collaboration can work in the care of geriatric patients in an acute setting from the perspective of the professions involved. Methods: The research question is answered using a qualitative approach. For this purpose, a systematic literature review, individual interviews and focus group interviews were conducted with representatives of the professions of medicine, nursing and physiotherapy, as well as with other healthcare professions in the context of “good practice” cases. Results: Specific aspects of interprofessional collaboration in the context of geriatric patient care are based on the presence of multimorbidity, cognitive limitations and reduced independence on the part of patients. In a permanent multi-professional care team, it is important to agree on a common goal and implement it through structured daily plans. The communication factor plays a special role due to the characteristics of geriatric patients. A team dynamic is required that takes each member seriously and supports them in order to be able to react appropriately to challenging situations in patient care. Conclusion: Despite literature-based knowledge regarding the influencing factors of interprofessional collaboration in the clinical setting, continuous reflection on patient-specific characteristics and needs is required in practice to be able to organize collaboration in the interprofessional team effectively and efficiently.
