Frühgeborene entwickeln häufig eine Anämie und benötigen wiederholt Erythrozytenkonzentrate. Derzeit werden adulte Spenderzellen verwendet, obwohl neonatale Erythrozyten eine physiologisch geeignetere Alternative sein könnten. In dieser Studie wurden adulte und Nabelschnur-Erythrozyten über 28 Tage in sieben Additivlösungen (SAGM, AS1, AS3, AS5, AS7, PAGSM, PAG3M) im Small-Scale-Modell gelagert. Hämolyse, Glukoseverbrauch, Laktatbildung und Morphologie wurden im Verlauf analysiert. Die Additivlösung determinierte Hämolyse und Metabolismus maßgeblich, während die Zellherkunft kaum eine Rolle spielte. SAGM induzierte in beiden Gruppen die höchste Hämolyse, wohingegen alkalische und phosphathaltige Medien (AS7, PAGSM, PAG3M) die Lagerungsqualität verbesserten. Die metabolischen Parameter spiegelten diese Hämolysemuster wider. Im Gegensatz dazu zeigten sich morphologisch deutliche Unterschiede: CB-Erythrozyten wiesen spezifische strukturelle Veränderungen auf. Da die Hämolyse in CB-Proben nicht mit der Formation von Sphäro-Echinozyten korrelierte, weisen biochemische und morphologische Lagerungsschäden hier eine teilweise Entkopplung auf. Zusammenfassend ist die Additivlösung der Hauptfaktor für die metabolische Stabilität, während CB-Erythrozyten ein eigenständiges morphologisches Verhalten zeigen. Da für adulte Zellen optimierte Bedingungen auch bei CB-Zellen überzeugten, unterstützt dies deren translationale Anwendbarkeit.

Preterm infants frequently develop anaemia and require repeated red blood cell (RBC) transfusions. Currently, adult donor erythrocytes are used for this purpose. However, neonatal erythrocytes may represent a more physiologically appropriate alternative. In this study, isolated adult and umbilical cord blood (CB) erythrocytes were stored for 28 days in seven different additive solutions (SAGM, AS1, AS3, AS5, AS7, PAGSM, and PAG3M) using a small-scale ex vivo storage model. Haemolysis, glucose consumption, lactate production, and morphological changes were assessed over time. Storage solution had a strong impact on haemolysis and metabolic parameters, whereas the effect of cell origin was generally minor. SAGM consistently induced the highest haemolysis in both groups, while alkaline and phosphate-containing solutions (AS7, PAGSM, PAG3M) showed improved storage performance. Biochemical parameters followed patterns similar to haemolysis, supporting the dominant role of additive solution composition. In contrast, morphological analysis revealed more pronounced differences between adult and CB erythrocytes. While haemolysis patterns were largely comparable, CB erythrocytes showed distinct morphological alterations. Notably, haemolysis did not consistently correlate with sphero-echinocyte formation in CB samples, indicating partially uncoupled biochemical and morphological additive lesions. These findings suggest that additive solution composition is the primary determinant of RBC storage quality, while CB erythrocytes display distinct morphological behaviour likely reflecting intrinsic biological differences. Importantly, conditions that performed well in adult erythrocytes also performed well in CB cells, supporting their translational applicability.
